High Cholesterol Self-Scheduler

Basic Criteria:

• Be 18 to 64 years of age

• Have fasted LDL-C ≥100 mg/dL 

• Be either treatment-naïve or off lipid-lowering therapy for at least 6 weeks 

• Have controlled blood pressure and diabetes 

• Have normal liver and kidney function 

Answers Must Be No: 

• Do you have a history of homozygous familial hypercholesterolemia? 

• Have you had a heart attack, heart failure hospitalization, unstable angina, coronary artery bypass graft, TIA, or stroke in the last 3 months? 

• Are you currently taking medications that interact with statins? 

• Do you have a personal or family history of hereditary muscular disorders? 

• Are you of Asian descent? 

Answers Must Be Yes: 

• Do you have high cholesterol? 

• Is your LDL-C ≥100 mg/dL? 

• Are you willing to take a statin or show proof of intolerance?
