Heart Failure and Obesity Self-Scheduler

Basic Criteria: 

• Be 18+

• Have a BMI over 30 

• Be diagnosed with HFpEF or HFmrEF for at least 6 months 

Answers Must Be No: 

• Do you have a history of any of the following in the past 2 months? 

• Myocardial infarction (MI), hospitalization for unstable angina, coronary artery bypass graft surgery or other major cardiovascular surgery, stroke, or transient ischemic attack (TIA) 

• Do you have Type 1 Diabetes?  

• Do you have a history of current or chronic Pancreatitis? 

• Do you have Obesity caused by endocrinologic disorders or monogenetic or syndromic forms of Obesity? 

• Do you have a history of active or unstable major depressive disorder (MDD) or other severe psychiatric disorder in the past 2 years?  

• Have you had cancer in the past 5 years except for:  non-melanoma skin cancer, in-situ carcinoma of the cervix, or in-situ prostate cancer 

• Are you the recipient of a heart transplant or any other major organ transplant (eg, lung, liver, bone marrow, renal), listed for heart transplant, or anticipated to receive chronic mechanical circulatory support or heart transplantation in the next year?  

• Have you been in a clinical research study in the past 3 months?  

Answers Must Be Yes:

• Have you been diagnosed with HFpEF or HFmrEF for at least 6 months?  

• If you are on Diuretic are you on a stable dose?  

• Additional criteria will apply
