Cardiovascular Disease Self-scheduler

Basic Criteria: 

• Be 18+ 

• Be on medication for Type 2 Diabetes & High Blood Pressure 

• Be diagnosed with Coronary Artery Disease, Cerebrovascular Disease, OR Peripheral Artery Disease 

Please note additional criteria will apply 

Answers Must Be No: 

• If female of childbearing potential, are you breastfeeding, pregnant, or planning on becoming pregnant? 

• Have you ever had an allergic reaction to lactose?  

• Have you ever been diagnosed with Heart Failure? 

• Are you receiving any of the following treatments or planning to during the study? 

• A direct renin inhibitor (e.g. aliskiren) 

• More than one ACEior ARB used simultaneously 

• Other aldosterone synthase inhibitors (e.g. baxdrostat ) 

• Systemic mineralocorticoid replacement therapy (e.g. fludrocortisone) 

• Do you have a known adrenal insufficiency (e.g. Addison disease) or Cushing’s syndrome? 

• Do you have a history of Ketoacidosis? 

• Have you been diagnosed with Type 2 Diabetes? 

• Have you had Cancer in the last 5 years? 

• Appropriately treated basal cell carcinoma of the skin, in situ carcinoma of uterine cervix, and low risk prostate cancer are allowed. 

• Have you had a Heart Attack, Stroke, or Mini Stroke in the last 3 months? 

• Have you taken part in a clinical trial in the last month? 

• Do you have HIV, Hepatitis A, or Hepatitis B? 

Answers Must Be Yes
