Basic Criteria:
- Be 18+ with at least one of the following.
-
- Coronary artery disease (CAD)
-
- Coronary artery score (CAC) >400
-
- History of surgical or percutaneous coronary revascularization procedure. Examples: Coronary Artery Bypass grafting, Percutaneous Coronary Interventintion, Endarterectomy, Transmyocardial laser Revascularization

- Stroke (ischemic, hemorrhagic or unknow type)
-
- Peripheral arterial disease (PAD) defined as either intermittent claudication within the last 12months OR prior nontraumatic amputation or peripheral procedure (stenting or surgical revascularization)
OR
- Be 55+ with high risk for CVD with at least 2 of the following
-
- Chronic kidney Disease eGFR <60
-
- AFIB and on medical therapy (anticoagulation or rate control)

- Most recent CAC score >100
-
- Type 1 or 2 diabetes, BMI≥ 30 or BMI ≥ 27 if East Asian, South East Asian or South Asian decent

- Treated high blood pressure, on a stable dose for 30days with a thiazide, thiazide-like or loop diuretic AND at least one other standard care of Blood pressure meds for any of these drug classes: ACE inhibitor or ARB, Calcium channel blocker, Beta blocker, MRA, Vasodilator (hydralazine, minoxidil, alpha blocker) or centrally acting antihypertensive medication (e.g. clonidine)

- Blood pressure range ≥ 145mmHg and <180mmHg Systolic
Answers Must Be No:
- If female of childbearing potential, are you currently pregnant, breastfeeding, or planning to become pregnant?
- In the past 12months have you had any alcohol or substance use disorder?
- Are you currently taking both an ARB and an ACE inhibitor (either as a single medication or part of combination medications?
- Is your high blood pressure because of other conditions including but not limited to history of renovascular hypertension, primary aldosteronism, pheochromocytoma, Cushing syndrome, or aortic coarctation?

- Exception: high blood pressure due to CKD.
- Do you have a condition where blood pressure drops significantly when a person stands up from a sitting or lying position?
- If diabetic, have you had an A1C >10 % in the past 2 months?
- Have you lost >10% of weight in the past 3months? Patients on drugs that cause significant weight loss like GLP-1 must be on stable dose for at least 3months.
- Do you have any history of or current liver cirrhosis?
- Do you have any active HIV infection? Exception: Patients on antiretroviral clinical stable and compliant with treatment for at least 6months
- Have you received any investigation product in the past 30days?
- In the past 3 months, have you used a potassium binder for the treatment of hyperkalemia?
- Do you have any sever respiratory disease, NYHA class IV heart failure, cancer in the past 5 years or any other condition that could limit participation for the duration of the study? Exception: non-melanoma skin cancer
- Have you undergone major organ transplant or anticipate doing so during the study?
- In the past 6 months, have you been treated or anticipate being treated with any therapeutic agent that targets AGT? Examples: Cozaar, Benicar, Atacand, Diovan, Avapro, Micardis ,Edarbi
- Do you have any current or history of severe intolerance (e.g. angioedema, recurrent hyperkalemia, recurrent acute kidney injury) to an ARB or ACE inhibitor other than cough?
- Have you had any significant CV events like heart attack, stroke, revascularization procedure or been hospitalized for heart failure in the past 60 days?
- Do you have severe aortic stenosis, a condition where the aortic valve controlling blood from the heart to the body becomes narrowed and does not open properly?
- Are you currently placed in an institution based on an official or court order?
Answers Must be YES
- Are you able to sign consent and comply with all study requirements?
- If female of childbearing potential, do you agree to use a highly effective method of contraception as listed below from at least 1 prior completed menstrual cycle through 12months after last dose administration?

- Oral, Implantable, transdermal, injectable methods associated with inhibition of ovulation

- Intrauterine device (IUD)
-
- Intrauterine hormone-releasing system (IUS)

- Bilateral tubal occlusion

- Have a partner who is vasectomized or sterile due to medical cause. Must bring documentation.

- Sexual abstinence if preferred and usual lifestyle
- If female of non-childbearing potential, do you meet one of the following requirements?

- Postmenopausal defined as no cycle for 12 months without an alternative medical cause.

- Documented: hysterectomy, bilateral salpingectomy, bilateral oophorectomy
