Bacterial Vaginosis Self-Scheduler

Basic Criteria  

• Age 14 or older 
• Must be assigned female at birth and has a vagina
• Presents with at least one (1) of the symptoms of vaginitis, including but not limited to: 
  • abnormal vaginal discharge, vaginal odor genital itching or irritation, pain/discomfort during sexual intercourse or urination, edema, OR Erythema 

Answers Must be NO 

• Are you currently menstruating? 

• Have you used any intravaginal products, vaginal deodorants, or douching in the last 48 hours? 

• Are you unable or unwilling to provide the appropriate required specimens (Vaginal swab)? 

• Do you have any medical conditions or bleeding disorders that may put you at risk for bleeding from collection of samples? 

• Have you previously enrolled in this study? 

Answers Must be YES 

• Are you willing to provide medical and/or sexual history required for this study? 
• If you are aged 14-17, do you have a legal guardian/parent who will be willing to sign an informed consent form? 
• Do you have sufficient medical and/or sexual history to document the information required for this study 
• Are you willing to take a sample/swab yourself here in our office?
