Flu Vaccine Self-Scheduler

Basic Criteria 

• Age 65 or older

Answers Must be NO 

• Are you currently confined to bed by sickness or old age? 
• Have you received a flu shot in the past 6 months? 
• Are you planning to get a flu shot anywhere else during the next 6-8 months? 
• Have you been in any other research studies with medications or vaccines in the past month? 
• Are you planning to join any other research studies in the next 6-8 months? 
• Do you take high-dose steroids like prednisone – 20 mg or more daily for more than 2 weeks at a time? 
• Do you take any chemotherapy or medications that suppress your immune system? 
• Have you had radiation treatment in the past 3 months? 
• Have you received any blood transfusions, blood/plasma products or IV immunoglobulin treatments in the past 6 months? 
• Have you ever had a severe allergic reaction to a flu shot – like trouble breathing, swelling, or needing emergency treatment? 
• Do you have any severe allergies to vaccines or vaccine ingredients like eggs? 
• Have you ever been diagnosed with Guillain-Barré syndrome – that’s a rare condition that can cause muscle weakness and paralysis? 
• Do you have multiple sclerosis, transverse myelitis, or any other nerve conditions that cause inflammation? 
• Do you have any current infection or had a fever of ≥100.4°F in the past 3 days?
• Do you have Celiac disease, Crohn’s disease, Ulcerative Colitis, Hashimoto’s, type 1 diabetes, Grave’s disease, Psoriasis, RA, Lupus, encephalitis, myelitis, MS, Narcolepsy, Vitiligo, or any other autoimmune diseases? 
• Do you have any medical conditions that would prevent you from getting a shot in your arm? 
• Do you have any conditions that would prevent you from having blood drawn? 
• Do you work for Seqirus pharmaceutical company, or is anyone in your immediate family involved with this research?
• Have you already participated in this specific flu vaccine study before? 

Answers Must be YES 

• Are you able to make your own medical decisions without a guardian? 
• Are you willing and able to comply with all study procedures and follow-up requirements? 
• Are you available for the full 6-8 month study duration? 
• Do you have reliable phone access for study calls and follow-up? 
• Are you willing to come to the clinic for all required study visits? 
• If you developed cold or flu symptoms during the study, would you be willing to come in for an extra visit so we can test you?
