Basic Criteria 

• Ages 18+ 
• BMI ≥25 (Click here to calculate)

 Answers Must Be No 

1. Have you been diagnosed with cirrhosis, congestive heart failure, HIV, or have untreated Hepatitis B or C?
2. Have you had cancer in the past 5 years (excluding skin cancers)? 
3. In the past year have you had any severe untreated psychiatric conditions or attempted suicide? (Treated conditions are okay. Untreated mild conditions that don’t affect daily life are okay.)
4. Have you abused alcohol in the past year? 
5. Are you currently enrolled in a clinical trial study involving drugs or device?

Answers Must Be Yes 

1. Are you able to read and understand a consent form independently?
